Daily co-trimoxazole is WHO-recommended standard care for pregnant women with HIV in malaria-endemic areas, but resistance of Plasmodium falciparum to antifolates threatens their effectiveness. Investigators in Kenya and Malawi assessed whether adding monthly dihydroartemisinin–piperaquine is more effective than daily co-trimoxazole alone. Pregnant women with HIV on dolutegravir-based combination antiretroviral therapy (cART) were randomized to daily co-trimoxazole plus monthly dihydroartemisinin–piperaquine (daily for 3 days) or daily co-trimoxazole plus monthly placebo. The primary endpoint was maternal or placental malaria from 2 weeks after the first study dose to delivery.
