Hospitalized people with HIV (PWH) in sub-Saharan Africa have high risk for death within 1 year of discharge. Investigators in Tanzania conducted a randomized trial to assess outcomes of the Daraja intervention, which consisted of five 45-minute sessions administered by a social worker beginning in the hospital and continuing for 3 months after the patient was discharged home. A total of 500 PWH who were hospitalized and antiretroviral therapy (ART) naive (80%) or off therapy for >7 days were randomized 1:1 to the intervention or enhanced standard of care (eSOC). The eSOC arm received predischarge HIV counseling and a warm handoff to the HIV clinic.
