Recent trials have shown benefit for selected patients with basilar artery occlusion (BAO) treated with endovascular thrombectomy (EVT). However, these trials enrolled mostly patients with small amounts of infarcted tissue at the time of treatment. Whether EVT is useful for patients with less-favorable imaging is unclear. These authors conducted a retrospective study of patients with BAO from multiple centers in Korea from August 2008 through April 2022. They assessed patients with a posterior circulation acute stroke prognosis early computed tomography score (PC-ASPECTS) of 6 or less, indicating the presence of at least moderate early ischemic changes.
