Clinicians caring for solid organ transplant recipients (SOTRs) with COVID-19 need information about their patients' risk for severe disease progression to provide optimal advice regarding early antiviral use and risk-reduction strategies. Now, a longitudinal cohort study of 1975 SOTRs with diagnoses between March 2020 and May 2023 shows how outcomes and risk factors for COVID-19 progression have evolved over the pandemic's course. Participants were temporally stratified by SARS-CoV-2 variant into one wildtype/Alpha/Delta wave and five Omicron waves.
