Pure autonomic failure (PAF) can lead to one of the neurodegenerative alpha-synucleinopathies. In this multicenter, longitudinal cohort study, researchers examined clinical features associated with phenoconversion to Parkinson disease (PD), dementia with Lewy bodies (DLB), or multiple system atrophy (MSA) in 209 participants with pure autonomic failure (64% men; median age, 70 years). Diagnosis of PAF, PD, DLB, or MSA was based on established criteria. Clinical assessments included motor, sleep, olfactory, cognitive, and autonomic measures at enrollment and at yearly follow-up visits.
