Several clinical trials have shown improved outcomes with the use of endovascular thrombectomy (EVT) for acute stroke in patients with large volumes of ischemic tissue at baseline. These trials have varied with respect to the imaging methods used and the size of the pretreatment ischemic core. Now, researchers have compared EVT plus medical therapy to medical therapy alone in patients who could be treated within 6.5 hours of stroke onset and had a large amount of ischemic tissue (defined by an Alberta Stroke Program Early Computed Tomography Score [ASPECTS] of ≤5). The primary outcome was the modified Rankin scale (mRS) score and the major safety outcome was all-cause mortality, both at 90 days.
