In addition to being spread through contaminated food and water, Shigella infections can be sexually transmitted as has been documented among men who have sex with men (MSM). Investigators in Chicago identified a cluster of nine cases with extended-spectrum beta-lactamase (ESBL) Shigella sonnei during the third quarter of 2022; all nine patients were men, six of whom reported attending an LGBTQ+ festival. Diarrhea was the most common symptom, with two patients requiring hospitalization for dehydration. In five patients who had antimicrobial susceptibility testing, resistance to ampicillin, azithromycin, ceftriaxone, ciprofloxacin, and trimethoprim-sulfamethoxazole was noted.
