Strategies to Manage Nonresponse to Hepatitis B Vaccine

In persons living with HIV who did not respond to prior vaccination, seroprotection was higher with a CpG-adjuvanted vaccine than with an alum-adjuvanted vaccine.

Persons living with HIV (PLWH) are at elevated risk for infection with hepatitis B virus, complications of infection, and low seroprotection (defined as HB surface antibody level >10 mIU/mL after HB vaccination). To evaluate seroprotection of three immunization strategies in previously vaccinated PLWH who have no current or past evidence of seroprotection, investigators conducted an open-label, international study in which they randomized 561 participants (median age, 46; median CD4+ count, 638 cells/µL; 94% had HIV RNA <40 copies/mL) to receive two doses of CpG-adjuvanted vaccine (2HepB-CpG), three doses of CpG-adjuvanted vaccine (3HepB-CpG), or three doses of alum-adjuvanted vaccine (3HepB-alum).
