Do Older STEMI Patients with Cardiogenic Shock Benefit from Microaxial Flow Pump Use?

An exploratory analysis of randomized data demonstrates higher mortality and less clear benefit of mAFP use in older patients with STEMI-related cardiogenic shock.

The DanGer Shock trial established that microaxial flow pump (mAFP) use decreases all-cause mortality in certain patients presenting with ST-segment elevation myocardial infarction (STEMI) and cardiogenic shock (CS; NEJM JW Cardiol Apr 8 2024 and N Engl J Med 2024; 390:1382). In an exploratory secondary analysis, the investigators examined whether this mortality benefit extends to older patients.

In the open-label trial, 355 adults with STEMI and CS (defined as hypotension, an elevated lactate, and left ventricular ejection fraction <45%) were randomized to mAFP placement plus standard care (including a revascularization attempt on all participants) or standard care alone.
