Does P2Y12 Inhibitor Pretreatment Improve Outcomes in Non–ST-Segment Elevation Acute Coronary Syndromes?

In patients managed with an early invasive approach, P2Y₁₂ inhibitor pretreatment was not associated with reduced ischemic events or increased major bleeding.

P2Y₁₂ inhibitor agents are sometimes administered prior to coronary angiography in patients with non–ST-segment elevation acute coronary syndromes (NSTE-ACSs), but evidence of benefit is mixed. In this observational study, researchers evaluated trends and clinical outcomes of P2Y₁₂ inhibitor pretreatment in the U.S. in patients with NSTE-ACSs (defined as non–ST-segment elevation myocardial infarction [NSTEMI] or unstable angina) who underwent coronary angiography within 24 hours of hospital arrival.

Among 110,000 patients (mean age, 64 years; 33% women; 86% white; 93% with NSTEMI), P2Y₁₂ inhibitor pretreatment decreased from 24% in 2013 to 12% in 2023. Using a hierarchical regression model, the authors found significant variability in P2Y₁₂ …
