Several studies of vitamin D supplementation in multiple sclerosis (MS) have not demonstrated benefit on disease activity but left open the possibility of a benefit on MRI (e.g., NEJM JW Neurol 2024 Jan 30 and Brain 2024; 147:1206). Investigators conducted this multicenter randomized trial in patients diagnosed with clinically isolated syndrome within the previous 90 days, based on not fitting 2010 McDonald diagnostic criteria. Of these participants, 89% would have met the 2017 McDonald criteria for MS. Participants received 100,000 IU of cholecalciferol (156 participants) or placebo (147 participants) orally every 2 weeks for up to 24 months.
