Long-distance running has become more popular in the past two decades in the United States. Using a well-established marathon and half-marathon registry, investigators prospectively assessed sudden cardiac arrests (incidence, cause, and need for cardiopulmonary resuscitation [CPR]) for runners in a 2000–2009 cohort compared with those in a 2010–2023 cohort.
Among 29.3 million race finishers in the 2010–2023 cohort, 176 sudden cardiac arrests occurred (0.54 per 100,000 participants), similar to the incidence rate in the 2000–2009 cohort. However, cardiac death incidence per 100,000 participants declined significantly, from 0.39 in the earlier cohort to 0.20 in the newer cohort.
