The 23-valent pneumococcal vaccine has been recommended for HIV-positive patients in the USA since 1989 based on the high incidence of invasive pneumococcal disease in this population and the apparent safety of the vaccine. However, efficacy data have been derived only from case-control studies, and studies of the pneumococcal vaccine's protective effect in HIV-negative populations have produced mixed results. This randomized, double-blind, placebo-controlled study conducted in Uganda raises further concern about the vaccine's value.
