In the absence of clinical-trial data, current recommendations on antiretroviral therapy (ART) initiation in HIV-infected infants (<12 months) are based largely on clinical opinion and drug availability. As a result, available guidelines vary substantially from organization to organization. The U.S. Department of Health and Human Services recommends that clinicians “consider” immediate treatment for all asymptomatic infants, whereas the World Health Organization recommends treatment only when certain immunologic or clinical criteria are reached. The Paediatric European Network for Treatment of AIDS recognizes either of these practices as reasonable options and additionally introduces virologic parameters into their proposed algorithm.
