Despite widespread recommendations, patients and providers have yet to embrace the use of antiretrovirals for nonoccupational postexposure prophylaxis (PEP) of HIV (ACC Feb 4 2008). Although effective, the dual-NRTI combination recommended in all PEP guidelines (AZT/3TC; Combivir) is difficult to tolerate, presumably because of the AZT component. Consequently, many recipients do not complete the prescribed 28-day course. To address this problem, investigators are exploring the safety and tolerability of alternative PEP regimens, such as those that contain tenofovir.
