The optimal time to start antiretroviral therapy (ART) remains one of the major unanswered questions in the current treatment era. One of the arguments against early use of ART has been concern about earlier development of drug-related toxicities. In the absence of randomized clinical trials on this topic, data from cohort studies can provide useful clues. In the HIV Outpatient Study, investigators assessed the incidence rates of three common comorbidities — peripheral neuropathy, anemia, and renal insufficiency — among 2165 participants who were followed for at least 3 years, on average, after starting ART.
