The Evolution of HIV Tropism in Individual Patients

Among HIV-infected men in the MACS cohort, CXCR4-using virus emerged well before the CD3⁺ inflection point and the development of late-stage HIV disease.

Understanding viral tropism is critically important now that HIV treatment options include the CCR5 inhibitor maraviroc. To date, however, our knowledge of this topic has remained rudimentary. For example, we know that the presence of CXCR4-using virus is associated with accelerated declines in CD4-cell counts, but we do not know how, when, or why viruses shift from using CCR5 exclusively to also using CXCR4.

To examine the emergence and persistence of CXCR4-using virus among individual patients over time, investigators reviewed long-term follow-up data from 67 men who seroconverted before 1995 and were enrolled in the Multicenter AIDS Cohort Study (MACS).
