Understanding viral tropism is critically important now that HIV treatment options include the CCR5 inhibitor maraviroc. To date, however, our knowledge of this topic has remained rudimentary. For example, we know that the presence of CXCR4-using virus is associated with accelerated declines in CD4-cell counts, but we do not know how, when, or why viruses shift from using CCR5 exclusively to also using CXCR4.
