Treatment of HIV-infected pregnant women, coupled with postnatal antiretrovirals for their infants, has led to a dramatic decrease in mother-to-child transmission of HIV. Lopinavir/ritonavir is the only “preferred” protease inhibitor in pregnancy, according to U.S. perinatal guidelines, and is frequently used in newborns as well, even though it is not licensed for use in infants younger than 14 days. Recent reports have described possible systemic and cardiac lopinavir/r toxicity in preterm infants, prompting an FDA warning, but no population-based data exist.
