On October 14, 2011, the U.S. Department of Health and Human Services released updated HIV treatment guidelines that reflect new data on agents for treating antiretroviral-naive adults and adolescents. The first-choice (“preferred”) regimens have not changed, but some reassortment has taken place among the second-tier (“alternative”) and third-tier (“acceptable”) options.
The new nonnucleoside reverse transcriptase inhibitor rilpivirine is now an “alternative” option for initial therapy. It is available both alone and in a combination tablet with tenofovir/FTC.
The combination of abacavir/3TC + either raltegravir or ritonavir-boosted darunavir has been elevated from a regimen that “may be acceptable” to an “alternative” choice.
