Most HIV-infected patients reside in resource-limited settings without access to routine viral-load and drug-resistance testing. In India, as in Africa, nonnucleoside reverse transcriptase inhibitor (NNRTI)-based regimens are used as first-line treatment, and protease inhibitor (PI)-based regimens are used as second-line treatment. Little is known, though, about second-line treatment failure in India in terms of rates, resistance profiles, and optimal management.
