In April 2026, the U.S. FDA approved islatravir/doravirine (Idvynso) for the treatment of HIV in people who already have virologic suppression on a stable antiretroviral regimen and who have no known history of virologic treatment failure or resistance to doravirine.
Standard first-line therapy for HIV consists of an integrase inhibitor plus one or two nucleoside reverse transcriptase inhibitors. However, some people receiving these regimens have adverse effects from the integrase inhibitor. Islatravir is a novel nucleoside analogue that inhibits reverse transcriptase by blocking translocation. Doravirine is a previously approved nonnucleoside reverse transcriptase inhibitor.
