The authors describe a cluster of papules on an erythematous base along the mastectomy scar of a woman with a history of breast cancer. Histopathology of the lesion confirmed metastasis. While cutaneous metastasis is rare, and other etiologies may cause a similar rash, clinicians can’t ignore the emperor of all maladies especially when it’s already lurking in the background. In women, cutaneous metastases are most often caused by breast cancer and, unfortunately, usually are a poor prognostic sign.
