Palmoplantar targetoid lesions with vesicles and oral mucosal erosions should raise suspicion for erythema multiforme (EM). Although EM can be medication-induced or idiopathic, herpes simplex virus remains one of its most common trigger, which was confirmed in this case by polymerase chain reaction testing. EM is more than skin deep: It can be accompanied by systemic features such as fever and malaise and can recur, usually in the setting of herpes simplex virus reactivation.
