Patients in intensive care units (ICUs) often experience delirium brought on by pain and opioid therapy. The role of adjunctive agents such as acetaminophen in combating delirium is uncertain. In a retrospective, single-center study of 18,000 adults admitted to a Boston ICU for at least 48 hours between 2015 and 2024, researchers examined whether acetaminophen exposure affected delirium burden. Exposure was defined as at least 2 g daily of oral or intravenous acetaminophen on any ICU day.
Delirium occurred in a significantly lower percentage of acetaminophen recipients than nonrecipients (25% vs. 33%). The finding remained significant after adjustment for demographics, illness severity, and analgesic exposure.
