Management of secondary hyperparathyroidism due to chronic kidney disease includes vitamin D replacement, dietary phosphate restrictions, and phosphate binders with meals. If substantial metabolic abnormalities persist, the calcimimetic medication cinacalcet or parathyroidectomy might be considered; however, whether medication or surgery should be favored is uncertain.
In this retrospective study of 12,000 patients with kidney-related secondary hyperparathyroidism (13% of whom were treated surgically), researchers compared outcomes between ≈1200 patients who underwent subtotal parathyroidectomy and ≈1200 propensity-matched patients who received cinacalcet.
