Current guidelines for managing acute ischemic stroke generally recommend intravenous thrombolysis (IVT) within 4.5 hours of onset, followed by mechanical thrombectomy if indicated. However, most studies of this approach didn’t include patients with basilar artery occlusion (BAO), and those that did evaluated administration of IVT at a comprehensive stroke center. In practice, many patients receive IVT at an outside hospital before being transferred to such centers.
