Basilar artery occlusion (BAO) is highly disabling and often fatal without successful recanalization. Although endovascular thrombectomy within 24 hours improves outcomes, it is not widely available. Thus, the standard of care in many settings is intravenous thrombolysis within 4.5 hours of symptom onset or, if that is not possible, anticoagulation or antiplatelet therapy. Now, researchers in China have conducted a phase 3, open-label, multicenter trial to test whether tenecteplase given up to 24 hours after symptom onset improves outcomes compared with standard care.
