Finerenone — a selective nonsteroidal mineralocorticoid-receptor antagonist — was approved by the U.S. FDA in 2021 to lower risk for progression of chronic kidney disease (CKD) in patients with type 2 diabetes; the drug is not approved for use in other patients with CKD. Blockade of the mineralocorticoid pathway is postulated to limit adverse effects that include sodium retention, inflammation, and fibrosis — in theory, this mechanism should be beneficial in nondiabetic CKD as well.
