Many standard therapies used in the out-of-hospital setting have never been shown to be safe or effective. Some that seemed sound have not proven to be beneficial, such as the pneumatic antishock garment, prehospital fluid resuscitation for penetrating injuries, and transcutaneous pacing for out-of-hospital bradyasystolic cardiac arrest. Randomized trials, when possible, are important to evaluate standard therapies. In this prospective, randomized, double-blind trial, investigators evaluated out-of-hospital administration of benzodiazepines to 205 patients with status epilepticus. The study was conducted under a waiver of informed consent; oversight was provided by multiple levels of regulatory review and safety analyses.
