Recently, results from two studies on preventing contrast-induced nephropathy have been published. The findings might change our view about the best prophylactic therapy for patients who require radiographic procedures.
In a prospective randomized single-center Italian study, 502 patients with creatinine clearances <60 mL/minute were enrolled before they underwent planned angiographic procedures. Patients were randomized to receive isotonic saline infusions (1 mL/kg/hour for 12 hours before and after their procedures) or sodium bicarbonate infusions (154 mEq/L, infused at 3 mL/kg for 1 hour before their procedures and at 1 mL/kg/hour for 6 hours afterward).
