The PIRO system, suggested as a classification scheme for sepsis, stratifies patients based on Predisposing conditions, the nature and extent of the Insult, the nature and magnitude of the host Response, and the degree of sepsis-induced Organ dysfunction. Now, for the first time, researchers have assessed the usefulness of a score inspired by this concept. They developed a PIRO system for severe community-acquired pneumonia (CAP), assigning 1 point each for chronic obstructive pulmonary disease/immunocompromise or age >70 (“predisposition”), bacteremia or multilobar pneumonia (“insult”), shock or hypoxemia (“response”), and acute renal failure or acute respiratory distress syndrome (“organ dysfunction”).
