Current guidelines suggest that when candidemia develops in a patient with an indwelling central venous catheter (CVC), the CVC should be removed. This recommendation is based on small, nonrandomized studies, and a randomized, controlled trial (RCT) is unlikely to be performed. To determine whether early CVC removal (within 24 or 48 hours after antifungal treatment initiation) is associated with improved outcome, researchers analyzed data from two large multicenter studies that examined the efficacy of antifungal therapies for candidemia. The current study — and the previous two — were conducted with funding from Astellas Pharma, which manufactures AmBisome (liposomal amphotericin B) and Mycamine (micafungin).
