In Puerto Rico, dengue is endemic, with transmission throughout the year but highest from June through November. After an increase in dengue-negative acute febrile illnesses there, investigators instituted enhanced surveillance for such conditions at the emergency department in a large tertiary care hospital. Patients with fever of unknown origin for 2 to 7 days were enrolled from September 29 through December 18, 2009.
Of the 284 patients enrolled, 49% had influenza (2009 pandemic H1N1 in 136 of 138) and 11% had dengue; 1 patient had influenza/dengue coinfection.
