Is there a role for glucose-insulin-potassium (GIK) therapy in the management of ST-segment-elevation MI (STEMI)? Researchers conducted a double-blind, placebo-controlled trial in which 20,201 STEMI patients (mean age, 59) at 470 centers worldwide were randomized to receive either high-dose GIK infusion for 24 hours plus usual care or usual care alone. All subjects presented within 12 hours of symptom onset, 74% underwent thrombolysis, and 9% underwent primary PCI. Most subjects participated in a concurrent trial of reviparin versus placebo (Journal Watch Cardiology Mar 18 2005).
