Clopidogrel improves outcomes in most patients with acute coronary syndromes, but poor responsiveness to clopidogrel is common and is associated with recurrent ischemic events. In the Pharmacogenomics of Antiplatelet Intervention (PAPI) Study, researchers measured platelet aggregation in 429 Amish adults who had taken clopidogrel for 7 days. The investigators performed a genomewide association study and genotyped the loss-of-function cytochrome P450 (CYP) 2C19*2 variant. They tested their findings and assessed the variant's relation to cardiovascular events in an independent U.S. population sample of 227 individuals undergoing percutaneous coronary intervention.
