Use of RAS Antagonists Reduces Mortality in Some Patients with HFPEF

In a large registry study, significant improvements in survival were evident in patients with EFs less than 50%.

To assess the role of renin-angiotensin system (RAS) antagonists in treating heart failure with preserved ejection fraction (HFPEF), investigators identified 16,216 individuals with HFPEF (defined as an EF of ≥40%) who were enrolled in a Swedish HF registry between May 11, 2000, and October 10, 2011. Of these patients, 9103 received angiotensin-converting enzyme (ACE) inhibitors, 3172 received angiotensin-receptor blockers (ARBs), and 268 received both.

Crude 1-year survival in the overall HFPEF cohort was 86% in recipients of RAS antagonists and 69% in nonrecipients; 5-year survival was 55% and 32%, respectively. In an age- and propensity-matched cohort involving 6658 HFPEF patients, 1-year survival was 77% in treated patients versus 72% in…
