Hydroxychloroquine and chloroquine are useful agents for cutaneous lupus erythematosus and other dermatologic diseases. Development of an irreversible, sight-threatening retinopathy is a major concern with long-term use. However, early recognition and cessation of therapy often halt progression of retinopathy before it becomes clinically noticeable to the patient. Information in the Physicians' Desk Reference and package inserts suggests that recipients should have quarterly ophthalmologic examinations. This frequency has been questioned, and in the era of cost-benefit analysis and evidence-based medicine, reexamination of the optimal timing and type of testing is needed.
