White-light endoscopy traditionally has been considered to be inaccurate for differentiating adenomas from hyperplastic polyps. Chromoendoscopy, particularly when combined with magnification endoscopy, can effectively delineate the pit patterns of polyps and thereby predict histology. The pits on hyperplastic polyps are uniformly sized and spaced (types 1 and 2), whereas those on adenomas are elongated and irregularly sized (3L and 4) or occasionally very tiny (3S). Narrow-band imaging (NBI) enhances the visibility of pit patterns and also can distinguish adenomas by demonstrating microcapillaries on the polyp surface.
