Current guidelines for postpolypectomy surveillance make the following recommendations: (1) Patients with one or two small tubular adenomas on their most recent examination should undergo follow-up colonoscopy in 5 to 10 years; (2) patients with 3 to 10 adenomas, any adenoma with villous elements or high-grade dysplasia, or an adenoma measuring ≥1 cm should have another exam in 3 years; and (3) patients with more than 10 adenomas, and those with large sessile adenomas removed piecemeal, should have a follow-up in less than 3 years. Would risk for recurrence be better determined on the basis of two previous colonoscopies, rather than just the most recent one?
