Tumor necrosis factor (TNF) antagonists have been implicated in drug-induced liver injury (DILI). However, the mechanism of injury and patient outcomes both remain unclear. In the current case series, researchers described cases of DILI likely to be associated with a TNF antagonist. They included 6 patients identified from a U.S. DILI registry and 28 patients identified from a literature search. Results were as follows:
The implicated drug was infliximab in 26 patients, etanercept in 4, and adalimumab in 4.
Severity scores (obtainable for 30 patients) were mild, 20; moderate, 8; and severe, 2.
Seventy-five percent of patients had a hepatocellular injury pattern.
Peak alanine aminotransferase (ALT) levels ranged from 140 U/L to 2250 U/L.
