Investigators from the population-based Rotterdam Study used transcranial Doppler ultrasonography to assess the relation between middle cerebral artery hemodynamics and stroke risk in 2022 people who were older than 60 and free of stroke at baseline. Average follow-up from time of ultrasound to stroke, death, or study end was 5.1 years. The middle cerebral artery was insonated at or near a depth of 50 mm. If blood flow velocity (BFV) was available for both sides, the average value was used. Cerebral vasomotor reactivity (VMR) was defined as the percentage increase in mean BFV while inhaling 5% CO2, divided by the absolute increase in end-tidal CO2.
