Initial Presentation
A 39-year-old woman presented 1 year ago with fatigue and jaundice. She was found to have hepatitis C and elevated liver enzymes. Anti-smooth antibody testing was positive (1:20). Test results for antinuclear antibody and HIV infection were negative. The patient also had histories of hypertension, IV drug abuse, and sexual relations with an IV drug user in the remote past. She reported no tobacco use and occasional alcohol use. A liver biopsy showed severe steatosis with focal pericellular fibrosis, consistent with superimposed steatohepatitis. The findings were suspicious for autoimmune hepatitis as opposed to nonalcoholic steatohepatitis. The patient was treated conservatively and discharged.
