Statins are a mainstay of secondary prevention after vascular events such as ischemic stroke, transient ischemic attack, and myocardial infarction (MI). However, secondary analyses of two large trials showed an increased risk for intracerebral hemorrhage (ICH) in patients with a prior stroke who were treated with simvastatin or atorvastatin. Interpretation of these findings is challenging, particularly for treating patients who have indications for statin treatment (e.g., prior MI) and a prior ICH. To address this issue, researchers performed a decision analysis accounting for variations in indication for statin use (primary or secondary prevention), location of prior ICH, and risks for cardiovascular and cerebrovascular events.
