Sporadic inclusion body myositis (sIBM) is the most common acquired muscle disease in people older than 50, with an estimated prevalence of 0.5 to 1 case per 100,000 people. IBM is traditionally considered one of the inflammatory myopathies, along with polymyositis (PM) and dermatomyositis (DM). Sporadic IBM is refractory to the immune suppressive treatment that is generally effective for PM and DM. For this reason and because sarcoplasmic and myonuclear protein aggregates accumulate within muscle in sIBM, there has been much debate about whether sIBM is autoimmune in origin or primarily a degenerative disorder of muscle.
