Sumatriptan is now a popular parenteral treatment for migraine headache in the emergency department, but it is contraindicated in certain patients and doesn't always work. Prochlorperazine and metoclopramide are also used for migraine, but the two dopamine antagonists have not been compared head-to-head. This trial randomized 75 ED patients with migraine to receive IV prochlorperazine (10 mg), IV metoclopramide (10 mg), or saline placebo. The patients graded their symptoms on a scale of 0 to 10. Median pain scores were about 8 in each group at baseline; at 30 minutes, the median score was significantly lower with prochlorperazine (1) than with metoclopramide (4) or placebo (6).
