Despite enthusiasm for treating depression in patients with coronary artery disease (CAD), there are no data on the safety or efficacy of selective serotonin reuptake inhibitors in patients with recent unstable CAD. This international, multisite, double-blind study, sponsored by the maker of sertraline, involved 369 patients (64% men; 77% white) with major depressive disorder (MDD) who had presented with myocardial infarction or unstable angina during the preceding 30 days. The patients were randomized to receive 24 weeks of sertraline (50 to 200 mg daily) or placebo; exclusion criteria included serious noncardiac disease, uncontrolled hypertension, psychiatric comorbidity, psychotropic medication use, and recent psychotherapy.
