Maternal Asymptomatic Group B Strep Bacteriuria Can Lead to Chorioamnionitis

Early treatment with antibiotics may reduce transmission of this devastating infection to newborns.

Intrapartum antibiotics are recommended for all women colonized with group B streptococci (GBS) to reduce transmission of this serious infection to infants, but there is no consensus about the management of maternal asymptomatic GBS bacteriuria during early pregnancy. Investigators conducted a retrospective cohort study of all women with asymptomatic GBS bacteriuria (of any colony count) before 20 weeks’ gestation, seen at a Pittsburgh hospital over a period of more than 2 years. They compared this group to pregnant women with negative urine cultures to determine whether antibiotic treatment early in pregnancy is associated with improved pregnancy outcomes — specifically, decreased risk for chorioamnionitis.
