Minimally invasive parathyroidectomy (MIP) for primary hyperparathyroidism can be performed under cervical block anesthesia as an outpatient procedure. Preoperatively, imaging to identify abnormal glands is performed; intraoperatively, peripheral venous parathyroid hormone levels are measured to document the expected fall after abnormal glands are removed.
In this report, a Yale surgeon who pioneered MIP presents his experience with 613 standard parathyroidectomies, mainly performed before 1998, and 1037 MIPs done since then. About 10% of MIP cases ultimately required conversion to general anesthesia for various reasons.
