Should hydrochlorothiazide or chlorthalidone be used preferentially when treating patients with uncomplicated hypertension? In this observational cohort study, researchers in Ontario, Canada, compared the composite outcome of death from all causes or hospitalization for heart failure, stroke, or myocardial infarction in patients (age, >65) who were newly treated with either agent between 1993 and 2010. During that interval, 11,000 patients received chlorthalidone, and 640,000 received hydrochlorothiazide. Using propensity score matching, the authors compared a 10,000-patient chlorthalidone group and a 20,000-patient hydrochlorothiazide group who were clinically similar to the chlorthalidone group in most baseline characteristics.
