Intracerebral hemorrhage usually has catastrophic consequences: Hemorrhage-related death or long-term disability occur in more than half of those affected. Recombinant activated factor VII (rFVIIa; NovoSeven) is a potent procoagulant that enhances coagulation at the site of vascular injury; it has been effective in controlling bleeding in people with congenital and acquired disorders of hemostasis, such as hemophilia and circulating anticoagulants. In an earlier trial of rFVIIa in patients with intracerebral bleeding, university and industry investigators found that this agent attenuated hematoma growth and lowered mortality at 90 days from 29% to 18% (N Engl J Med 2005; 352:777).
